FEES

The athletic fee is $35 per player per sport. The deposit is refundable when uniform is returned at the end of the season.

FORMS & LIABILITY WAIVERS

Prior to the first practice, a parental consent form/liability waiver and concussion/sudden cardiac arrest information form must be completed and turned into the school office. When filling out the form, please use the following information:

Type of event – [The sport the student will be participating in]
Location – Star of the Sea Gym and other locations for games
Individuals in charge – Coaches
Date and time of departures/return – varies
Mode of transportation – parent arranged carpools

VOLUNTEERING

If you are interested in volunteering (by becoming a coach, carpooling students, becoming a team manager, keeping score, operating the clock, etc.), please contact the school office or the athletic director. Coaching counts for 25 volunteer hours automatically.
